מחקר בתחום הטיפולים הדינמיים-חווייתיים

על חשיבות המחקר והבסיס המחקרי של הגישה: לטיפולים דינמיים-חווייתיים (EDT) ישנה תמיכה אמפירית חזקה ומוצקה. ניסויים קליניים מבוקרים עם הקצאה אקראית (RCTs) – הנחשבים ל”סטנדרט הזהב” (Gold Standard) בהערכת יעילותם של טיפולים – נערכו עבור קשת רחבה של הפרעות דיכאון, חרדה, תסמינים סומטיים (גופניים) והפרעות אישיות.

לאורך השנים, חשיבות המחקר בתחום זה הוכיחה את עצמה  – לא רק לצורך תיקוף ההתערבויות הקליניות והבנת מנגנוני השינוי, אלא גם לשם הרחבת היישום של הגישה לקבוצות מגוונות של מטופלים. הנגשת הידע המחקרי נועדה להעניק למטפלים ולמטופלים כאחד ביטחון בדרך הטיפולית, הנשענת על פרקטיקה מבוססת-ראיות (Evidence-Based Practice).

מחקרי ליבה ומטה-אנליזות (Meta-Analyses)

גוף המחקר ב-EDT גדל ומתפתח בקצב מהיר במיוחד. כדי להנגיש את הידע ולהקל על ההתמצאות, סקירת הספרות בעמוד זה מבוססת בעיקרה על מחקרי רוחב וסקירות מקיפות של חוקרים מובילים בתחום – ובהם אלן אבאס, ג’ון מולדר-בראון ופיטר לילינגרן – המאגדים עשרות מחקרים קליניים לבחינת יעילותה של הגישה בהשוואה לשיטות טיפול אחרות. 

מטא-אנליזה של מחקרים אקראיים מבוקרים (Meta-analysis of EDT RCTs)

סקירה מקיפה זו מהווה אבן דרך מחקרית. מחקר מקיף של Peter Lilliengren ועמיתיו (עודכן לאחרונה ב-2025) הסקירה החדשה כוללת 57 מחקרים אקראיים מבוקרים (RCTs) שפורסמו עד שנת 2024, ומקיפה 4,330 מטופלים. הממצאים מדגימים כי לגישות ה- EDT כהתערבויות טרנס-דיאגנוסטיות, יעילות השווה לטיפולים נתמכי-ראיות אחרים, עם יתרון משמעותי בשמירה על תוצאות הטיפול (Follow-up) לטווח הארוך.

הפרעות חרדה (Anxiety Disorders)

על פי הגישה, חרדה היא לרוב תגובה פיזיולוגית לא-מודעת המופעלת כאשר רגשות קונפליקטואליים (כגון כעס או אשמה) מתעוררים ומאיימים לפרוץ למודעות. הטיפול מסייע למטופל לזהות את האופן שבו החרדה מתבטאת בגוף, לווסת אותה, ולחוות בביטחה את הרגשות העמוקים המסתתרים תחתיה. מחקרים מצביעים על כך שהתערבות ממוקדת זו מובילה להפחתה מהירה ומשמעותית בסימפטומים החרדתיים, עם תוצאות שנשמרות היטב לאורך זמן.

דיכאון

בגישת ה-EDT, דיכאון מומשג פעמים רבות כתוצאה של רגשות מורכבים ומוצדקים (כעס וזעם) שהופנו כלפי פנים (Retroflected anger). כאשר רגשות עוצמתיים, במיוחד כלפי דמויות התקשרות, נחסמים – המטופל נוטה להפנות אותם אל עצמו ולהעניש את עצמו, מה שמוביל לסימפטומים דיכאוניים, לביקורת עצמית קשה ולתחושת ייאוש. מחקרים קליניים מראים כי סיוע למטופל להרגיש ולתעל רגשות אלו כלפי חוץ משחרר את המנגנון המדכא, ומביא להטבה ניכרת ומהירה בדיכאון.

מפגש הערכה והתערבות מקיף (Trial Therapy)

מפגש ה-Trial Therapy הוא מפגש טיפולי ראשון וארוך (לרוב כ-3 שעות ויותר, כפי שנהוג ב-ISTDP קלאסי), שנועד לאפשר למטפל ולמטופל לעשות עבודה מקיפה, לבחון את התגובה להתערבות, ולבסס אבחנה ומוקד טיפולי מדויק. המחקרים מצביעים על כך שמפגש ייחודי זה אינו רק כלי אבחוני, אלא בעל ערך תרפויטי משמעותי בפני עצמו, המזרז את הברית הטיפולית ואף מוביל להטבה סימפטומטית כבר מתחילת הדרך.

מחקרים אלו מתפתחים החל ממחקרים שבוצעו על ידי מטפלים מומחים בלבד,ועד לשחזור עצמאי של תוצאות אלו באמצעות מחקרי Trial Therapy ב-EDT. בנוסף, מחקר רחב היקף שנערך על מדגם גדול וכלל מספר רב של מטפלים שונים הוכיח כי תוצאות המפגש קשורות באופן ישיר להצלחה בחשיפת הלא-מודע (Unlocking the Unconscious) – ממצא מרתק בפני עצמו.
